The No Surprises Act & Good Faith Estimate

Under section 2799B-2(d) of the Public Health Service Act (PHS Act), enacted as part of the No Surprises Act, all patients have a right to receive a Good Faith Estimate explaining how much your medical care will cost.


The Good Faith Estimate is a provision of the No Surprises Act that requires all healthcare providers to give clients an itemized estimate of expected non-emergency healthcare services via a standardized format. This helps ensure transparency of costs for patients to prevent surprise billing or unexpected charges.

On your estimate, you will see a breakout of anticipated therapy or assessment sessions, as well as the total anticipated cost for services. The Good Faith Estimate itself is not a contract with Quest, only an estimate of services to better help you understand the cost of receiving healthcare.

Additionally:

  • You have a right to dispute a bill that substantially exceeds your Good Faith Estimate ($400 or above). For more information, please visit https://www.cms.gov/nosurprises/consumers/understanding-costs-in-advance

  • When provided a Good Faith Estimate, you will have an option to download the form. Be sure to save a copy of your estimate.

  • The estimate will include all services related to treatment, but will not include peripheral items, such as letter, no-show, late cancellation, or other fees that cannot be anticipated when the estimate is provided.
