Psychological Testing / Evaluation

 

What is a Psychological Evaluation?

The purpose of testing is often to clarify a diagnosis (e.g. learning challenges, depression, anxiety, trauma, Bipolar, Borderline Personality Disorder) using empirically validated measures along with structured interview questions. The formal report produced from testing can be used in academic settings for accommodations, in medical settings for medication management, and in conjunction with therapy to aid in personal or treatment needs. Psychological evaluations are a great way to aid in your own self-discovery process! Testing is available to anyone, both children and adults; no referral is necessary!


Assessments We Provide


Full Psychological Evaluation (Ages 6 - Adulthood) $3270

ADHD / Anxiety / Depression / Social Anxiety / Autism / Bipolar / Oppositional Defiant Disorder / Behavioral Problems / Trauma / PTSD / Eating Disorders / Personality Disorders and Emerging Traits

This is Quest’s most requested “deep dive” evaluation (approx. 15-25 pages)

 

Purpose may be:

  • To inform the course of treatment

  • For medication management

  • For therapy recommendations

  • For home, school, and/or vocational recommendations

  • To increase a deeper self-understanding

Includes:

  • 90 minute Clinical Interview and follow-up interviews as necessary

    • Mood Measures

    • Personality Measures

    • Cognitive Measures

    • Trauma Measures

    • Projective Measures

    • Broadband Measures

    • Adaptive, Substance, & Eating Measures, as applicable

    • Consultation with treatment team (e.g. therapist, prescriber, teacher)

  • 90 minute Feedback/Results Appointment

 

Educational Evaluation (Ages 6 - College) $3270

ADHD / Anxiety/ Depression/ Social Anxiety / Autism / Learning Disability

 

Purpose may be:

  • For use in academic settings to set up 504 plan or IEP

  • For home, school, and vocational recommendations

  • To increase a deeper understanding of yourself/your child

Includes:

  • 90 minute Clinical Interview and follow-up interviews as necessary

    • IQ Testing

    • Achievement Testing

    • Cognitive Measures

    • Broadband Measures

    • Adaptive and Mood Measures, as applicable

    • Consultation with treatment team (e.g. therapist, prescriber, teacher)

  • Peer-reviewed and co-written by clinical and school psychologist at Quest

  • Approx. 15-25 pages

  • 90 minute Feedback/Results Appointment

 

Psychoeducational Evaluation (Ages 6 - College) $4270

ADHD / Anxiety / Depression / Social Anxiety / Autism / Bipolar/ Oppositional Defiant Disorder / Behavioral Problems / Trauma / PTSD / Eating Disorders / Personality Disorders and Emerging Traits for 9+ youth AND Learning Disability

 

Purpose may be:

  • For use in academic settings to set up 504 plan or IEP

  • For home, school, and vocational recommendations

  • To increase a deeper understanding of yourself/your child

Includes:

  • 90 minute Clinical Interview and follow-up appointments as necessary

  • 3 Sessions of Assessment:

    • IQ Testing

    • Achievement Testing

    • Cognitive Measures

    • Broadband Measures

    • Adaptive, Mood, & Eating Measures, as applicable

    • Consultation with professionals within treatment team (e.g. therapist, prescriber, teacher)

  • Peer-reviewed and co-written by clinical and school psychologist at Quest

  • Approx. 20+ pages

  • 90 minute Feedback/Results Appointment

 

We do NOT offer forensic, custody, capacity, bariatric, or fitness for duty assessments.

We currently are NOT able to provide ADOS testing.
